Sliced Apples Recalled in Canada for Listeria Risk, 1 Illness Reported

Sun Rich Fresh Foods Inc. is recalling from the marketplace sliced apples and products containing sliced apples produced in its Brampton, Ontario, facility due to possible Listeria monocytogenes contamination. One illness has been reported associated with consumption of these products, according to the Canadian Food Inspection Agency (CFIA).

Consumers should not consume, and distributors, retailers and food service establishments such as hotels, restaurants, cafeterias, hospitals and nursing homes should not sell or use, the recalled products described below:

The public is being advised to check to see if the recalled products are in homes or establishments. This recall was triggered by CFIA test results. The agency is conducting a food safety investigation, which may lead to the recall of other products. If other high-risk products are recalled, CFIA will notify the public through updated Food Recall Warnings.

Food contaminated with Listeria monocytogenes may not look or smell spoiled but can still make you sick. Symptoms can include vomiting, nausea, persistent fever, muscle aches, severe headache and neck stiffness.

Pregnant women, the elderly, and people with weakened immune systems are particularly at risk from Listeria infection. Although infected pregnant women may experience only mild, flu-like symptoms, the infection can lead to premature delivery, infection of the newborn, or even stillbirth. In severe cases of illness, people may die.

Hy-Vee Recalls Summer Fresh Pasta Salad for Potential Listeria Contamination

Hy-Vee Inc. of West Des Moines, IA, has recalled its Hy-Vee Summer Fresh Pasta Salad sold in its stores’ kitchen department cold cases and salad bars.

The product was recalled after Hy-Vee was notified that the frozen vegetables used to make the ready-to-eat pasta were potentially contaminated with Listeria monocytogenes. The frozen vegetables were produced by Inventure Foods Inc., which recalled some of its products last week after Listeria was found in its facility in Jefferson, GA.

(2 lb.) clear plastic containers. A light tan scale-produced label with the product name, weight and price would have been affixed to the container.

The recalled product would have been available in a limited number of stores between April 9, 2015, and April 27, 2015.

Hy-Vee has since pulled the ready-to-eat Summer Fresh Pasta Salad from its distribution channels and the stores in Illinois, Iowa, Minnesota, Missouri, Nebraska and South Dakota to which it was distributed. All stores that received the product have been instructed to dispose of it.

Hy-Vee stated that it had not received any complaints to date associated with its Summer Fresh Pasta Salad.

Customers who purchased Summer Fresh Pasta Salad from the Hy-Vee kitchen department cold case or salad bar between April 9, 2015, and April 27, 2015, should dispose of the product or return it to the store for a refund.

Waymouth Farms Recalls Raw Pine Nuts for Possible Salmonella Contamination

Waymouth Farms Inc. of New Hope, MN, is recalling RAW PINE NUTS in various sizes because the product has the potential to be contaminated with Salmonella. The product was distributed nationwide through retail stores and mail order under the Good Sense® brand. Salmonella bacteria can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Healthy persons infected with Salmonella often experience fever, diarrhea (which may be bloody), nausea, vomiting and abdominal pain.

In rare circumstances, infection with Salmonella can result in the organism getting into the bloodstream and producing more severe illnesses such as arterial infections (e.g., infected aneurysms), endocarditis and arthritis.

Skyline Provisions Recalls 1,029 Pounds of Beef Products for E. Coli Risk

Skyline Provisions Inc. of Harvey, IL, is recalling 1,029 pounds of beef products contaminated with E. coli O157:H7, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced today.

Produced between April 15-25, 2015, the following products are subject to recall:

  • 17.5 boxes of Aurora Packers Intact Beef Round Flats

On April 15, 2015, Skyline sold the product under their D&S label (Establishment number: 19300), ground and tested 1.5 cases of the product. On April 21, 2015, these products were found positive for E. coli O157:H7.

The remaining intact products were sold to Jack & Pat’s Old Fashioned Market in Chicago Ridge, IL, where the product was ground and sold in various amounts of ground chuck patties, ground chuck, ground round, sirloin patties and porter house patties.

groundbeefchub-406FSIS discovered the problem during a routine sampling program. Neither FSIS nor the company have received any reports of illnesses associated with consumption of this product. However, FSIS and the company are concerned that some product may have been sold and stored in consumers’ freezers.

E. coli O157:H7 is a potentially deadly bacterium that can cause dehydration, bloody diarrhea and abdominal cramps 2-8 days (3-4 days on average) after exposure to the organism.

While most people recover within a week, some develop a type of kidney failure called hemolytic uremic syndrome (HUS). This condition can occur among persons of any age but is most common in children younger than 5 and older adults. It is marked by easy bruising, pallor, and decreased urine output. Persons who experience these symptoms should seek emergency medical care immediately.

FSIS advises all consumers to safely prepare their raw meat products, including fresh and frozen, and only consume ground beef that has been cooked to a temperature of 160 degrees F. The only way to confirm that ground beef is cooked to a temperature high enough to kill harmful bacteria is to use a food thermometer that measures internal temperature.

Salmonella Dog Chews Recalled for Salmonella Threat to People

Dog chews sold under the Nylabone Puppy Starter Kit brand are being recalled for possible Salmonella contamination with a potential risk to humans. The Neptune, NJ-based TFH Publications, Inc./Nylabone Products issued the recall on April 22.

“Salmonella can affect animals ingesting the product and there is risk to humans from handling contaminated products, especially if they have not thoroughly washed their hands after having contact with the products or any surfaces exposed to these products,” the recall notice said.

Symptoms of a Salmonella infection in humans include: nausea, vomiting, diarrhea or bloody diarrhea, abdominal cramping and fever. Salmonella can result in more serious ailments, including arterial infections, endocarditis, arthritis, muscle pain, eye irritation, and urinary tract symptoms.

Anyone exhibiting these signs after having contact with this product should contact their healthcare providers.

Pets with Salmonella infections may be lethargic and have diarrhea or bloody diarrhea, fever, and vomiting. Some pets will have only decreased appetite, fever and abdominal pain. Infected but otherwise healthy pets can be carriers and infect other animals or humans. Anyone with a pet with such symptoms should contact a veterinarian, especially if the animal consumed the recalled product.

The recalled Puppy Starter Kit consists of one lot of dog chews that were distributed nationwide, to Canada, and through one domestic online mail order facility. The product comes in a 1.69 oz. package marked with Lot #21935, UPC 0-18214-81291-3, located on the back of the package, and with an expiration date of 3/22/18 also stamped on the back of the package.

The potential for contamination was noted after routine testing by the company revealed the presence of Salmonella in one lot of 1.69 oz. packages of the Puppy Starter Kit.

Peas, Corn, and Broccoli Recall For Listeria Contamination

Today’s Harvest Field Peas with Snaps, Silver Queen Corn, and Broccoli Florets with the sell by dates 04/21/16 are being recalled by manufacturer Greystone Foods, LLC and Publix Supermarkets because of possible Listeria contamination.

Birmingham, AL-based Greystone on April 16 shipped the potentially contaminated product to a Public Supermarkets warehouse in Lakeland, FL, and the manufacturer does not think any contaminated products left the facility. Nevertheless, Publix is removing the brands from is retail shelves as a precaution.

Listeria is a sometimes deadly pathogen that is special threat for the elderly, young children, and anyone with a compromised immune system. It also known for causes miscarriages and stillbirths among pregnant women.

No other grocers are involved in the recall, according to the notice, which was released by the U.S. Food and Drug Administration (FDA).

The recalled products come in 32 ounce clear plastic bags and are located in the freezer section of Publix. The sell by date of 04/21/16 is printed along the bottom seal of the bag in black ink.

“The potential for contamination was noted after we were informed by our supplier that an area of their facility where the field peas with snaps were blended tested positive for Listeria monocytogenes on a non-food contact surface,” the Greystone recall notice says.

The company is confident all the recalled product are being quickly identified and removed from commerce. It said if any customers find packages with the sell by date of 04/21/16 we urge them to return them to the store where they were purchased for a refund.

Frozen Vegetables, Smoothie Kits Recalled After Listeria Found in Georgia Plant

Inventure Foods Inc. of Phoenix, AR, has announced a recall of certain varieties of its Fresh Frozen™ line of frozen vegetables as well as select varieties of its Jamba “At Home” line of smoothie kits due to finding Listeria monocytogenes in its Jefferson, GA, facility.

The Jamba “At Home” smoothies’ products being recalled are distributed to retail outlets, including mass merchandise stores and supermarkets east of the Mississippi River. Only specific Jamba “At Home” branded products are involved in this recall. No other Jamba® branded products are affected.

Jeni’s Splendid Ice Creams Recalls All Products, Temporarily Closes Stores Due to Listeria Risk

Jeni’s Splendid Ice Creams of Columbus, OH, announced Thursday that the company has initiated a recall of all ice creams, frozen yogurts, sorbets, and ice cream sandwiches for all flavors and containers because of the possible presence of Listeria monocytogenes.

The ice creams, frozen yogurts, sorbets, and ice cream sandwiches being recalled were distributed in the U.S. to retail outlets, including food service accounts and grocery markets, as well as online at jenis.com. This recall includes all products bearing the “Jeni’s” brand name.

The contamination was discovered in a sample randomly collected by the Nebraska Department of Agriculture.

Jeni’s Splendid Ice Creams is not aware of any illness reports to date related to the recalled products. However, out of an abundance of caution, Jeni’s Splendid Ice Creams stated that the company is taking this measure in order to ensure complete consumer safety.

“Our top priority is guaranteeing the safety of all consumers by taking every possible precaution,” said John Lowe, CEO of Jeni’s Splendid Ice Creams.

“We have called in experts to help us find the root cause,” Lowe said. “We will be working with our suppliers to determine if the bacteria was introduced by one of the ingredients we use. We will not reopen the kitchen until we can ensure the safety of our customers.”

Customers who have purchased any of the products are urged to dispose of them or return them to the store where they were purchased for an exchange or full refund and to consult with their physician regarding any medical questions.

This is the second recall announced this week involving ice cream and related products. On Monday, Blue Bell Creameries of Brenham, TX, announced that it was recalling all of its products made at all four of its plants due to the risk of Listeria contamination. There are 10 illnesses and three deaths to date potentially linked to Blue Bell products.

Listeria monocytogenes is an organism that can cause serious and sometimes fatal infections in young children, frail or elderly people and others with weakened immune systems. Although healthy individuals infected by Listeria may suffer only short-term symptoms such as high fever, severe headache, stiffness, nausea, abdominal pain, and diarrhea, infected pregnant women can suffer miscarriages or stillbirths.

Contaminated Fresh Spinach Outruns Confusing Recall Notices

Contaminated fresh produce often makes its way into consumers faster than the product can be effectively recalled.

An example of how this happens occurred earlier this month with fresh spinach produced by the Salinas, CA-based Taylor Farms — the winner for the past two years of the Safe Quality Foods (SQF) Institute’s “Primary Producer” award for food safety.

On April 2, Taylor Farms produced fresh spinach on three lines which, by April 7, was at a western Michigan distribution warehouse. Once it arrived at the warehouse, samples of the fresh spinach were taken by the Michigan Department of Agriculture and Rural Development (MDARD) from bagged product going out to schools, hospitals and restaurants, according to a MDARD spokesperson.

freshspinach-406The samples were taken for the department’s routine food safety assurance program and were tested for pathogens at the Geagley Laboratory in East Lansing, MI.

On April 13, results came back from the lab showing the spinach was positive for both Salmonella and non-shiga toxin producing E. coli. Because the product was involved in interstate commerce, MDARD’s Food and Dairy Division contacted the U.S. Food and Drug Administration (FDA), which then became the lead investigator for the incident.

The following day, April 14, Taylor Farms issued a voluntary recall for “certain lots codes of washed spinach.” The company stated that the product had tested positive “for a bacterial pathogen,” adding that, “The pathogen of concern is still unknown at this point … .” Taylor Farms also noted that it was “initiating the recall out of an abundance of caution.”

The first public report of the recall was issued not by Taylor Farms, but by Grand Rapids, MI-based Gordon Food Service, according to the news service covering the fresh produce industry known as The Packer. Gordon Food Service distributed the recall notice on April 14 using its website and the Taylor Farms letterhead to list two affected lots of spinach, 092A27 and 092A28.

A Taylor Farms employee with knowledge of the recall told Food Safety News that the codes mean that the contaminated spinach was all produced by separate lines during the day shift on April 2.

Next to chime in was the national commercial food distributor US Foods based in Rosemont, IL. On April 15, US Foods issued a “correction” on its website for the Taylor Farms recall, stating that it was for “Fresh, Flat Leaf Spinach due to possible contamination of Salmonella” being sold under both the Taylor Farms and Cross Valley Farms brands.

In addition, US Foods stated that one additional code was involved. This was for a third line, also from the day shift on April 2 — 092A24. (US Foods also has each code beginning with the letters YTF, while the Gordon Food Service left those off.)

In the 20 days since the Taylor Farms spinach was harvested in the Salinas Valley, no illnesses are known to be associated with the recall. Fresh spinach is typically consumed within two or three days of being purchased at the grocery store unless it is cooked and frozen for later use.

The two food service companies, and maybe others, distributed the spinach in several states, and it’s a good bet the great bulk of it was eaten by consumers who never heard about the recall.

The Class 1 recall, meaning there was an immediate health danger at hand, was never posted on FDA’s recall website in either its original or corrected form. FDA’s site is widely used by the food industry for such voluntary recalls.

The dueling Gordon Food Service and US Foods recall notices were confusing because Gordon apparently reported only on product it had from two lines, while US Foods had spinach to recall from three.

So while this fresh spinach recall wasn’t clear to the media or the public, it was a good illustration of how the fresh produce market moves faster than the recall mechanisms that are supposed to catch up with it.

Blue Bell Recalls All Products Made in All Company Facilities

Blue Bell Creameries of Brenham, TX, announced Monday that it is recalling all of its products currently on the market made at all of its facilities, including ice cream, frozen yogurt, sherbet and frozen snacks, because they have the potential to be contaminated with Listeria monocytogenes.

Blue Bell Creameries logoThe company stated that Monday’s decision was the result of findings from an enhanced sampling program which revealed that Chocolate Chip Cookie Dough Ice Cream half gallons produced on March 17, 2015, and March 27, 2015, contained the bacteria.

This means Blue Bell has now had several positive tests for Listeria in different places and plants and, as previously reported, five patients were treated in Kansas and three in Texas after testing positive for Listeria monocytogenes. Three of the five people sickened in Kansas died.

The company still does not know what the source of the contamination might be, but Blue Bell’s president indicated that everything was being done to determine what caused this first product recall in the the company’s 108-year history.

“We’re committed to doing the 100-percent right thing, and the best way to do that is to take all of our products off the market until we can be confident that they are all safe,” said Paul Kruse, Blue Bell CEO and president, in a 37-second video statement posted Monday on the company’s website.

“We are heartbroken about this situation and apologize to all of our loyal Blue Bell fans and customers,” Kruse said. “Our entire history has been about making the very best and highest quality ice cream and we intend to fix this problem. We want enjoying our ice cream to be a source of joy and pleasure, never a cause for concern, so we are committed to getting this right.”

The products being recalled are distributed to retail outlets, including food service accounts, convenience stores and supermarkets in Alabama, Arizona, Arkansas, Colorado, Florida, Georgia, Illinois, Indiana, Kansas, Kentucky, Louisiana, Mississippi, Missouri, Nevada, New Mexico, North Carolina, Ohio, Oklahoma,  South Carolina, Tennessee, Texas, Virginia, Wyoming and international locations.

“At every step, we have made decisions in the best interest of our customers based on the evidence we had available at the time,” Kruse said. “At this point, we cannot say with certainty how Listeria was introduced to our facilities and so we have taken this unprecedented step. We continue to work with our team of experts to eliminate this problem.”

Blue Bell is implementing a procedure called “test and hold” for all products made at all of its manufacturing facilities. This means that all products will be tested first and held for release to the market only after the tests show they are safe. The Broken Arrow, OK, facility will remain closed as Blue Bell continues to investigate.

In addition to the “test and hold” system, Blue Bell plans to implement additional safety procedures and testing including: expanding its system of daily cleaning and sanitizing of equipment, expanding the system of swabbing and testing its plant environment to include more surfaces, sending samples daily to a leading microbiology laboratory for testing, and providing additional employee training.

Blue Bell expects to resume distribution soon on a limited basis once it is confident in the safety of its products. However, the products could be off the market for two to three weeks.

Chicken Sausage Products Recalled for Possible Plastic Pieces

Kayem Foods of Chelsea, MA, is recalling approximately 59,203 pounds of fully cooked chicken sausage products that may be contaminated with pieces of plastic, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced Monday.

  • 12-oz. packages of Trader Joe’s brand “Sweet Apple Chicken Sausage” with the case code 9605 and use by/freeze by dates of “4 22 15,” “4 25 15,” and “4 29 15.”
  • 8-oz. packages of al fresco brand “Apple Maple Fully Cooked Breakfast Chicken Sausage” with the case code 9709 and use by/freeze by dates of “JUN 13 2015” and “JUN 20 2015.”

The products subject to recall bear the establishment number “P7839” inside the USDA mark of inspection. These items were shipped to retail locations nationwide.

The problem was discovered after the firm received complaints from two consumers who found small pieces of plastic in the product.

Neither the company nor FSIS has received reports of any injuries associated with consumption of these products. Anyone concerned about an injury or illness should contact a healthcare provider.

La Clarita Queseria Cheese Recalled for Risk of Staphylococcus Aureus Contamination

Consumers are being warned not to eat La Clarita Queseria Queso Fresco “Fresh Cheese” made by Queseria La Poblanita Inc. of New York, NY, due to possible Staphylococcus aureus contamination.

Officials from the New York Department of Agriculture and Markets stated there are no known illnesses to date associated with this product.

La Clarita Queso FrescoThe recalled fresh Spanish-style cheese is sold in 12-oz. plastic tub packages with a label declaring a plant # 36/8585 and a product lot code of MAY 13, 2015. The manufacturer presented on the package is: “Queseria La Poblanita Inc., 216 E. 117 St., New York, NY  10035.”

The product was distributed to stores and delis in the metropolitan New York area.

A routine sample of the cheese, collected by an inspector from the Department’s Division of Milk Control and Dairy Services on April 15, 2015, was subsequently tested by the New York State Food Laboratory and discovered to be contaminated with high levels of Staphylococcus aureus.

The manufacturer was notified of the positive test result and voluntarily recalled the product.

Consumers who have purchased the La Clarita Queseria Queso Fresco Fresh Cheese should return the product to the place of purchase or discard it.

Staphylococcus aureus is a bacterium that usually causes rapid food poisoning symptoms, including nausea, vomiting, retching, abdominal cramping, and prostration. In more severe cases, headache, muscle cramping and transient changes in blood pressure and pulse may occur.

Recovery generally takes two days, and everyone is susceptible. While it is rare, some people have died from such cases, particularly the elderly, infants, and those who were severely debilitated.

Washington Dairy Recalls Raw Milk and Cream After Positive Listeria Test

A raw milk dairy in Washington state has voluntarily recalled some of its raw milk and cream products after a recent routine test found Listeria monocytogenes bacteria in a sample of raw cream.


dairy-cows-lineup-350 Spanish Sonrise Dairy in Yacolt, WA, has recalled all of its raw cow’s milk and goat’s milk and cream with expiration dates between April 16-21, 2015, according to The Columbian newspaper in Vancouver, WA.


Products with those expiration dates should be thrown away, and anyone with questions about the recall can call the dairy at (360) 931-7283, between 8 a.m. and 5 p.m. PDT.


The recalled products were sold at Amboy Market, Sonrise Market in Dollars Corner, New Seasons Market in Vancouver, and Camas Produce and Arnada Naturals in Vancouver. The milk is sold in gallon and half-gallon plastic containers, and the cream is sold in pint and quart plastic containers.


The recall was announced as a precaution, and no milk samples tested positive for Listeria, according to news reports.


The dairy’s owners, Jose and Tina Rodriguez, said they pulled all their cow’s milk and cream from store shelves on Friday and retrieved the goat’s milk from stores on Monday.


Tina Rodriguez said this was the first positive test result for the dairy, which the couple opened in 2011. Because of the incident, they have reportedly decided to close the dairy.


Listeria infection is particularly dangerous for pregnant women and those with compromised immune systems.

Wegmans Thai Green Bean and Mango Salad

This delicious recipe comes from the Wegmans grocery store magazine, Menu. It adds bright color and cool fresh vegetables and fruit to your plate with fantastic flavor. The original recipe called for twice the green beans. The lemon grass paste is a Wegmans product called Gourmet Garden Lemon Grass Herb Blend, a mixture of lemon grass, canola oil, sea salt, and some whey, sold refrigerated in their produce section. I haven't worked with lemon grass much, so I don't know whether minced fresh lemon grass would suffice.
